Why Am I So Tired on Ozempic or Wegovy?

Fatigue on Ozempic is commonly traced to four contributors: lower B vitamin and magnesium intake from eating less, lower electrolyte intake (sodium, potassium, magnesium), blood sugar fluctuations, and iron deficiency. Why am I so tired on Ozempic or Wegovy?

Fatigue was reported by 11% of Wegovy users in clinical trials. It is a common complaint among GLP-1 patients and one of the least adequately explained by prescribers. There are at least four distinct mechanisms discussed.

Mechanism 1: Caloric deficit and energy production

Energy is produced at the cellular level through a process that requires B vitamins, magnesium and adequate caloric intake. A 2025 joint advisory from four professional societies (Mozaffarian et al., Obesity) reported reductions in caloric intake of 16 to 39 percent across the GLP-1 drug class. Mechanism 2: Electrolyte intake

Food accounts for approximately 20% of daily fluid and electrolyte intake. When food intake drops significantly, sodium, potassium and magnesium intake falls with it. Electrolyte imbalance can cause fatigue, cognitive fog and physical weakness. Many patients read GLP-1 dehydration as a medication side effect when lower electrolyte intake may be playing a part, and intake is something you can reasonably attend to.

Mechanism 3: Blood sugar fluctuations

GLP-1 medications affect insulin secretion and blood glucose regulation. Blood sugar fluctuations can occur particularly after eating, creating periods of low energy that feel like amplified post-meal crashes.

Mechanism 4: Iron deficiency

Reduced food intake during GLP-1 treatment frequently leads to reduced iron intake, particularly in women. Iron deficiency is associated with a heavy, physical exhaustion, reduced capacity for ordinary daily activity and cognitive slowing. If fatigue persists beyond week four of GLP-1 treatment, discuss iron testing with your healthcare provider. Bloodwork is the only way to know. The overlap with sleep disruption

GLP-1 fatigue and GLP-1 sleep disruption are related but distinct problems. Fatigue is primarily discussed in terms of nutritional intake and cellular energy.
