Why Am I So Tired on Ozempic or Wegovy?

Fatigue on Ozempic is caused by four distinct mechanisms: B vitamin and magnesium depletion from reduced food intake, electrolyte loss (sodium, potassium, magnesium), blood sugar fluctuations, and iron deficiency. It is a cellular energy deficit, not tiredness from lack of sleep. B vitamin supplementation and electrolyte replacement address the two most common underlying causes.

Why am I so tired on Ozempic or Wegovy?

Fatigue was reported by 11% of Wegovy users in clinical trials. It is one of the most common complaints among GLP-1 patients and one of the least adequately explained by prescribers. There are at least four distinct mechanisms driving it.

Mechanism 1: Caloric deficit and energy production

Energy is produced at the cellular level through a process that requires B vitamins, magnesium, and adequate caloric intake. When food intake drops by 16 to 39%, all three inputs drop with it. The result is a cellular energy deficit that more sleep does not resolve. See what nutrients Ozempic depletes for the full picture of micronutrient loss during GLP-1 treatment.

Mechanism 2: Electrolyte depletion

Food accounts for approximately 20% of daily fluid and electrolyte intake. When food intake drops significantly, sodium, potassium, and magnesium levels fall. Electrolyte imbalance is a direct cause of fatigue, cognitive fog, and physical weakness. Many patients mistake GLP-1 dehydration for a medication side effect when it is actually an electrolyte issue that is directly correctable.

Mechanism 3: Blood sugar fluctuations

GLP-1 medications affect insulin secretion and blood glucose regulation. Blood sugar fluctuations can occur particularly after eating, creating periods of low energy that feel like amplified post-meal crashes.

Mechanism 4: Iron deficiency

Reduced food intake during GLP-1 treatment frequently leads to reduced iron intake, particularly in women. Iron deficiency causes a heavy, physical exhaustion combined with reduced exercise tolerance and cognitive slowing. If fatigue persists beyond week four of GLP-1 treatment, discuss iron testing with your healthcare provider. See signs of nutrient deficiency on Ozempic for the full list of symptoms to watch for.

The overlap with sleep disruption

GLP-1 fatigue and GLP-1 sleep disruption are related but distinct problems. Fatigue is primarily driven by nutritional depletion and cellular energy deficit. Sleep disruption is driven by tryptophan depletion lowering serotonin and elevated cortisol from rapid weight loss. Both can co-occur and compound each other.
